At San Juan County Partnership, we walk alongside individuals and families who are experiencing homelessness or at risk of losing their housing, meeting people where they are and helping them find a path forward.

​

Through Permanent Supportive Housing, our HUD-funded Continuum of Care program, Rapid Rehousing, Homeless Prevention and Linkages, and additional support through Housing New Mexico/Mortgage Finance Authority, we offer a range of programs, each with its own eligibility guidelines, so more people in our community have a door to walk through.

 

And no one does it alone: every program includes case management, whether from our own team or through our behavioral health partners, so support doesn't end once someone has keys in hand.
